Today is the last day to get your blood drawn at your physician’s office or a participating LabCorp facility and to submit your online health questionnaire on the Bravo member portal. All full-time employees and spouses who plan to enroll in the Maritz plan will need to meet today’s deadline to be eligible to receive well credits and avoid the tobacco surcharge for the next calendar year.
