For additional details, refer to the Medical page of this website (under the Health tab).
Short-Term Disability
The Maritz short-term disability program provides weekly income protection to regular full-time employees during the first 26 weeks of illness, injury, or pregnancy. However, please note that benefits for normal pregnancies with no complications are limited to 6 weeks following the delivery date (or 8 weeks for a c-section).
Short-term disability benefits are payable effective the first day of absence if you are unable to work for more than 7 consecutive calendar days (6 or more business days). For absences lasting only 5 consecutive business days or less, refer to the policy for other absences.
Long-Term Disability
To protect you and your family from financial hardship in case of a long-lasting illness or injury, the long-term disability insurance plan provides meaningful income replacement to full-time employees after 180 days of disability.
File a Claim
Maritz is changing the administrator for our leaves of absence from Lincoln Financial to Voya starting September 1, 2024.
If your first day of disability leave was on or after September 1, 2024, you will file a new disability claim with Voya by phone at 1-888-973-3652 or online at www.trackingabsence.com/eep. If eligible, a FMLA claim will also automatically be opened for you.
If your first day of disability leave was prior to September 1, 2024, you will file your disability claim with Lincoln Financial by phone at 1-888-408-7300 or online at www.mylincolnportal.com (register as a new user first and enter Maritz as the company code when prompted). Your disability claim will continue to be administered by Lincoln Financial throughout the duration of your leave and will not transition to Voya on September 1. However, if your leave also qualifies under FMLA, the FMLA portion of your leave will be transitioned to Voya starting September 1. This means you will be communicating with Lincoln Financial for your disability claim/approval, and with Voya for your FMLA claim/approval. Refer to the Family & Medical Leave (FMLA) page of this website for additional details.
For further information regarding instructions for filing a disability claim and your responsibilities during the leave process, refer to the appropriate Instructions document in the PDFs section, depending on whether your leave is for maternity or other non-maternity medical reasons.
For additional information about short-term or long-term disability benefits, refer to the policy and other links in the PDF section for each benefit.
